The medical aptitude form is the official document that determines a worker’s aptitude to perform a given professional function. It is filled in and signed by an occupational doctor after an admission, periodic or occasional examination has been carried out in accordance with article 110 of Law no. 102/2009, of September 10.
If the result of the health check reveals that the worker is unfit, the occupational physician must indicate, where appropriate, other duties that the worker can perform.
Whenever the repercussions of the work and the conditions in which it is carried out prove to be harmful to the worker’s health, the occupational doctor must notify the person responsible for the occupational health and safety service and, if the state of health so warrants, request that the worker be accompanied by a doctor from the health center or another doctor indicated by the worker.
A copy of the medical aptitude form is sent to the company’s human resources manager.
It should be noted that the aptitude form may not contain any information involving professional secrecy.
